Quick Answer
  • Syphilis is a bacterial STI treated with penicillin injections — the exact dose depends on how long you've had it.
  • Symptoms change dramatically by stage: an early painless sore, then a rash, then often no symptoms at all for years.
  • Syphilis does not go away on its own — the sore heals, but the infection remains and can cause serious harm decades later if untreated.
  • Doxycycline is the main alternative for a penicillin allergy, but isn't used for neurosyphilis or during pregnancy.
  • A visit at Great Doctors USA costs a flat $39.99, with same-day physician review.

What Is Syphilis?

Syphilis is a bacterial sexually transmitted infection caused by Treponema pallidum. Unlike chlamydia or gonorrhea, syphilis progresses through distinct stages, with symptoms that can look completely different — or disappear entirely — as it advances. Syphilis medication (a penicillin injection) is highly effective, especially caught early, which is why testing matters even when a sore has already healed on its own.

The Stages of Syphilis

Syphilis symptoms change as the infection progresses — which is exactly why it's sometimes called "the great imitator."

Primary

A single painless sore (chancre) at the site of infection, usually appearing about 3 weeks after exposure. It heals on its own in 3–6 weeks — but the infection is still there.

Secondary

A skin rash (often on the palms and soles), fever, swollen lymph nodes, sore throat, patchy hair loss, or fatigue — usually weeks to months after the sore heals.

Latent

No symptoms at all. Syphilis can stay hidden in the body for years while still being present and, in later stages, still able to cause harm.

Tertiary (rare today)

If never treated, syphilis can eventually damage the heart, brain, nerves, and other organs — sometimes decades after the original infection.

A sore healing on its own does not mean syphilis is gone — the infection remains and can progress to later stages without treatment.

How Is Syphilis Spread?

Direct contact with a syphilis sore during vaginal, anal, or oral sex

Can be spread even without penetrative sex if a sore is present

A pregnant person can pass it to their baby during pregnancy, risking serious harm to the baby

It is not spread by hugging, sharing food or drinks, toilet seats, or casual contact

Common Question

Does Syphilis Go Away on Its Own?

No — not really. The initial sore heals on its own within weeks, which can make it seem like the infection resolved. But Treponema pallidum remains in the body and can silently progress to later, more serious stages. A penicillin injection is required to clear the infection; there is no home remedy or over-the-counter treatment that cures it.

What Happens If Syphilis Is Left Untreated?

Because symptoms can disappear on their own between stages, it's easy to leave syphilis untreated without realizing it. Over time, this can lead to:

Damage to the heart, brain, nerves, eyes, or other organs over time (tertiary syphilis)

Neurosyphilis — infection spreading to the brain or spinal cord, which can happen at any stage

Serious harm to a baby if passed during pregnancy, including stillbirth or birth complications

Increased risk of acquiring or transmitting HIV

Continued spread to sexual partners while untreated

Commonly prescribed
  • Benzathine Penicillin G — 2.4 million units by injection — a single dose for early-stage syphilis, or three weekly doses for late/unknown-duration syphilis
  • Doxycycline — 100mg twice daily for 14–28 days — alternative for a penicillin allergy, only if you're not pregnant and don't have neurosyphilis
Medication Details
  • Benzathine Penicillin G is first-line for every stage, and the only proven safe treatment during pregnancy
  • Early-stage syphilis: a single injection. Late-stage or unknown-duration: three weekly injections
  • Doxycycline is an alternative only for non-pregnant patients with a penicillin allergy
  • No over-the-counter syphilis medication exists — a prescription is required
  • Advanced-stage or neurosyphilis needs in-person IV treatment — we'll refer you if that's your situation

Avoid sexual activity until sores have completely healed and follow-up testing confirms treatment worked — and until any partners have also been treated.

Syphilis Testing: How It's Diagnosed

  • A blood test is the standard way to diagnose syphilis and identify which stage it's at
  • If neurological symptoms are present, a spinal fluid test may be needed — this requires in-person care
  • Results are typically available within a few days
  • The CDC recommends routine syphilis screening during pregnancy, and regular screening for those at higher risk

Not sure if you need a test?

Order a syphilis or full STI panel directly — results are reviewed by a physician, with treatment if you need it.

Syphilis and Pregnancy

Untreated syphilis during pregnancy can seriously harm a baby, including stillbirth or lifelong health problems. Penicillin is the only proven safe and effective treatment during pregnancy — even patients with a penicillin allergy typically need penicillin desensitization rather than an alternative drug. This is routinely screened for during prenatal care; let your physician know if you're pregnant during your intake.

Who's behind your care

Every visit is reviewed by a licensed physician

Not a script, not an algorithm — a real doctor reads your case, decides what's safe, and puts their license behind it.

Our medical review process

Every treatment plan is written and approved by a physician licensed in your state before it's sent to your pharmacy. If a case falls outside what's safe to treat online, we say so and refer you to in-person care.

Licensing & accountability

Our physicians hold active state medical licenses and are credentialed before seeing patients. We're a telemedicine platform, not an emergency service — for anything urgent, call 911. Prescriptions are sent electronically to a pharmacy of your choice; we never dispense medication directly.

Frequently asked questions

How is syphilis treated?

Syphilis is treated with an injection of benzathine penicillin G. The dose depends on the stage: a single injection for early-stage syphilis, or three weekly injections for late-stage or unknown-duration syphilis.

Is syphilis treated with a pill or a shot?

Primarily a shot. Penicillin injection is first-line for every stage. Doxycycline (a pill) is only used as an alternative for patients with a penicillin allergy who aren't pregnant and don't have neurosyphilis.

What are the stages of syphilis?

Primary (a painless sore), secondary (rash and flu-like symptoms), latent (no symptoms, can last years), and tertiary (organ damage, rare today because it takes years of untreated infection to reach this stage).

Can I have sex while being treated for syphilis?

No. Avoid sexual activity until any sores have completely healed and follow-up testing confirms the treatment worked, and until any partners have also been treated.

Does my partner need to be treated too?

Yes, if you've been sexually active with them recently. Without partner treatment, you can be reinfected even after your own treatment works. You can add partner treatment during checkout.

What happens if syphilis is left untreated?

Untreated syphilis can progress through stages and, years later, cause serious damage to the heart, brain, nerves, and other organs (tertiary syphilis), or spread to the brain or spinal cord (neurosyphilis).

Do I need a test before getting treatment?

Usually, yes. A blood test is typically needed to diagnose syphilis and determine what stage it's at, since that determines your treatment. We can order this through our lab services, or you can bring in prior results.

Is this private?

Yes. Your visit is private and HIPAA-compliant. Nothing is shared with employers, insurers, or family without your consent.

Will my insurance be billed?

No. Every visit is flat-rate, paid up front, at $39.99 — no insurance billing, no surprise costs after the visit.

How do you get syphilis?

Syphilis spreads through direct contact with a syphilis sore, usually during vaginal, anal, or oral sex. A pregnant person can also pass it to their baby during pregnancy.

Can syphilis go away on its own?

No. The initial sore heals on its own within weeks, which can make it seem like it resolved — but the infection remains in the body and can progress to more serious stages if untreated.

Is there an over-the-counter treatment for syphilis?

No. Syphilis requires a prescription antibiotic — there is no over-the-counter pill, cream, or medication that cures it. A physician has to confirm the stage and arrange the right treatment.

What if my case is more advanced (late-stage or neurosyphilis)?

If your case appears to be late-stage or involves neurological symptoms, your physician will tell you directly — this requires in-person evaluation and IV treatment, and you won't be charged for a visit we can't safely complete online.
